Can liposuction treat cellulite?
Cellulite is often present in the same areas that are prone to stubborn fat deposits, particularly the thighs and buttocks, so it’s understandable that patients often wonder if liposuction will eliminate cellulite—but the effects of liposuction on cellulite are limited.
Here, Cincinnati plastic surgeon Dr. Mark Mandell-Brown explains the relationship between liposuction and cellulite.
What is cellulite and what causes it?
Cellulite refers to the lumpy, dimpled appearance that occurs when fat deposits push through the fibrous bands of connective tissue, called septa bands, just below the skin’s surface. These bands become thicker and tighter over time, pulling the skin inward and creating the “cottage cheese” look we see as cellulite. This harmless condition is likely caused by a combination of several factors, including diet, lifestyle, hormones, and genetics.
Women are more likely to experience visible dimpling in the skin than men because their skin is thinner and because of how their fat cells and the orientation of their septa bands develop and interact.
The amount of excess fat you have does not necessarily determine the presence or severity of cellulite—even very thin people can have cellulite.
How liposuction works
Liposuction is a cosmetic procedure that removes excess fat deposits from farther below the skin’s surface in specific areas of the body. During liposuction, Dr. Mandell-Brown makes tiny incisions in strategic areas before inserting a thin tube called a cannula. Using the tumescent liposuction technique (which is the most comfortable and effective method) a numbing solution is injected into the fat layer and then broken up and suctioned out through the cannula. Once the ideal amount of fat is removed to contour and sculpt the desired area, Dr. Mandell-Brown carefully revisits treated areas to fine-tune the new shape to ensure a smooth, natural appearance.

Liposuction’s effect on cellulite
Liposuction’s ability to improve the look of cellulite is limited, as it does not remove the fat cells within the septa bands that cause the condition. Liposuction must be performed deep enough to prevent pronounced dimpling and other irregularities at the skin’s surface, so fat cells closer to the skin’s surface, where cellulite forms, are not suctioned.
In many cases, patients are no longer bothered by cellulite after liposuction because the underlying shape is smoother and more defined and thus can help the skin appear more even. However, the structural issues that cause cellulite can persist, meaning that liposuction alone is unlikely to provide a long-term solution.
Liposuction targets fat removal rather than the fibrous connective tissue responsible for cellulite.
Are there effective treatments for cellulite?
A nutrient-dense diet of whole foods and regular exercise can help reduce the amount of fat beneath the connective tissue and lessen the appearance of cellulite—much like liposuction. Topical treatments are largely ineffective, especially long-term, but there are treatments available that can help improve your skin’s appearance:
- Sculptra is a unique dermal filler that can fill in dimples while also stimulating collagen production to smooth and lift the skin.
- RF microneedling stimulates collagen to even skin texture and radiofrequency energy liquefies fat, making it a potentially effective solution to target cellulite.
- Some laser treatments are ideal for rejuvenating skin by promoting collagen remodeling and microcirculation, which helps improve the appearance of cellulite.
There are also surgical procedures specifically designed to break up the septa bands below the skin like Avéli® and Cellfina®, as well as injectable substances such as QWO® that help break down the fat. Dr. Mandell-Brown does not recommend these options for cellulite reduction as they can be associated with prolonged bruising and achieve only mild improvement.
